Write it as you would like it to appear. If your loved one had a nickname please show it in brackets like this example: Suzanne (Suzy) Mary Jones
If you would like this included, you can list as: *name of facility *at home *etc
If you'd like any details shared about the passing of your loved one, please include as much information as you'd like here. Some examples: *after a brief illness... *passed suddenly... *peacefully with family at his/her side...
List the close family survivors (first & last names) of the deceased and their relationship to the deceased. For example: *Parents *Spouse *Children *Sisters *Brothers For married children or siblings, list their spouse in parenthesis beside the name for example: sister, Mary Jo (Ben) Smith; brother, John (Anne) White
List the close family members who passed away prior to the death of your loved one. Include the relationship such as: *grandparent *parent *sister *brother *son *daughter
Select your preferred term of endearment that will be used to describe your loss.
