Memorial Page Order Form

The first step in preparing the information for the service is to create the memorial page. Information you will need to provide includes the obituary, date of birth, and date of death as well as a photo.

Providing us with a written obituary will help us prepare the memorial page for you. While it can be a daunting task, we’ve created some guidance that may help you overcome writer’s block.

Intro & Outro Media Order Form

As part of our service, we create a beautiful intro and outro screen to share as the gathering commences and at the conclusion. Please complete this form as soon as possible but no later than 48 hours prior to the gathering.

Digital Memorial Card – Prepaid with Original Booking

Digital Memorial Card – Pay with Order

If you have requested the optional Digital Memorial Card, you can place your order and customize it using this form. This form and related files are due a minimum of 4 days prior to the service.

Photo Video (Slideshow) Order

If you have requested the optional Photo Video you can place your order and customize it using this form. This form and the related files are due a minimum of 4 days prior to the service. The form pages provide instructions for formatting and uploading your files.

Send us Files

File Transfer

Use this link to send files. If you are sending files for a photo video, please number them in the order you would like them presented and include the name of the service. For example: janedoe-1.jpg, etc. Please place all the files in one folder on your computer and you can select and upload the folder rather than individual files. If you want to add additional photos, you can send a new folder.

Stand Alone Rehearsal

Book a Rehearsal

Our Professional Planned Service package includes a rehearsal for speakers which takes place 30 minutes before the event starts. If you would like to reserve a separate date and time for the rehearsal in advance, please use this link to reserve the time. The add-on fee for this separate rehearsal is $150.
